I’ve seen it. It’s basically a textbook. If you’re a professional athlete or really obsessed with every gram of macros, it’s great. For a casual-ish powerlifter, it’s probably overkill.

I’m sticking with 'The Lift Log' for now. I like the 'Canadian simplicity' of it—nothing you don't need, plenty of space for what matters.

What pen do you guys use? I know it sounds silly, but a good pen makes the tracking much more enjoyable. I use a Uniball Power Tank—it writes even if the paper is a bit damp or oily.

The Power Tank is a good call, NorthernLass. Ordinary ballpoints skip so much when the paper gets gym-chalk on it.

Thanks for all the suggestions! I think I'm going to look into 'The Lift Log' or maybe just go the 'Gymreapers' route. Anything to get me off my phone during sets.

One more tip: write your phone number on the inside cover. Most gym-goers are decent people and will call you if they find it in the locker room.
